Comment on Modus Therapeutics’ Q2 Report
Modus Therapeutics AB (”Modus” or ”the Company”) published its interim report for the second quarter of 2026 on August 26, 2026. Below are some of the points we have chosen to highlight in connection with the report:
- The Phase IIa study in CKD with anemia is progressing according to the communicated timeline
- Operating profit of SEK -4.6m, in line with the study’s progress
- Favorable safety profile and established Phase II dose in the SEVUSMART study in severe malaria
- Cash position of SEK 12.5m following the capital injection from TO 2025/2026
The Phase IIa Study is Progressing According to Plan
Modus states in the report that part 2 of the Phase IIa study in CKD with anemia is progressing as expected at the study centers in Verona and Pavia, that patient recruitment is assessed to be completed during Q4-26, and that top-line proof-of-concept data is expected around the turn of the year 2026/2027. The study, which was initiated in December 2025, evaluates the effects of repeated dosing of sevuparin on hemoglobin-related signals, hepcidin, and kidney related biomarkers in patients with advanced chronic kidney disease.
Analyst Group has consistently highlighted the Phase IIa study as Modus’ central value driver, where the results are assessed to be decisive for how the Company can continue developing its research portfolio. That the timeline essentially remains firm, although now divided in greater detail between recruitment and readout, is therefore, in Analyst Group’s view, an important message in the report, instilling confidence. Modus states at the same time that focus remains on the recruitment rate at the study centers, where measures are continuously evaluated to support a stable supply of patients.
Current treatment of CKD with anemia is based mainly on iron supplementation and erythropoiesis stimulating agents, whose effect is limited by the disease leading to elevated hepcidin levels, for example through increased inflammation, thereby impairing the body’s ability to utilize available iron. There are currently no approved treatments that specifically lower hepcidin. An outcome demonstrating a coherent effect from lowered hepcidin to improved iron availability and rising hemoglobin would, according to Analyst Group, constitute the first clinical evidence of sevuparin’s effect in patients within the main indication, thereby strengthening Modus’ position in the partner discussions held ahead of a potential out-licensing or sale of the Company.
The Cost Level Reflects the Study’s Progress
Operating profit during Q2-26 amounted to SEK -4.6m (-5.9), where research and development expenses amounted to SEK 2.8m (3.7). Compared with Q1-26, when operating profit amounted to SEK -3.3m, the cost base increased by SEK 1.4m. Modus states accrued costs attributable to the Phase IIa study as the explanation for the lower cost level compared with the previous year. The comparison quarter last year included the execution of part 1 of the Phase IIa study, whose patient recruitment was completed in July 2025, which is why the cost level during Q2-25 was above the Company’s normal level. The difference between the quarters is thus assessed to be mainly attributable to the respective phases of the study parts and to accruals, rather than to a changed activity level. The increase compared with Q1-26 reflects, according to Analyst Group, that part 2 is now being conducted at full intensity. Overall, we consider the cost development to be in line with the study’s progress and that Modus continues to operate with good cost control.
Capital Injection from TO 2025/2026 Secures Funding Through the Expected Data Readout
Cash and cash equivalents amounted to SEK 12.5m at the end of Q2-26, compared with SEK 6.9m at the beginning of the quarter. The strengthening is attributable to the exercise of warrants of series TO 2026, which were exercised to 94.8% and provided Modus with SEK 9.5m before issue costs of SEK 0.4m. Through the exercise, the number of shares increased by 27,073,298 to 148,701,791, corresponding to dilution of approximately 18% for shareholders who did not participate.
Cash flow from operating activities amounted to SEK -3.5m (-3.4) during the quarter. Applying a burn rate of approximately SEK 1.4m per month, in line with the average since the Phase IIa study was initiated, the cash position of SEK 12.5m is estimated to be sufficient until around February 2027, in line with the Company’s own communication regarding funding into Q1-27. Modus is thereby estimated to be financed through the expected data readout.
In addition, the cash position may be strengthened further during Q3-26. Warrants of series TO 2030 can be exercised annually during September through 2030 at a subscription price of SEK 0.40 per share, with the first exercise period falling between September 14 and 27, 2026. Upon full exercise of all outstanding warrants, Modus may be provided with a maximum of approximately SEK 15.2m before issue costs. According to the Company, such proceeds would be directed toward the next step in the development plan during 2027, for example linked to the continued collaboration in severe malaria and added runway for business development should proof-of-concept data prove favorable.
Scientific Basis in Severe Malaria Strengthened Through Submitted Manuscript
During Q2-26, Modus announced that Professor Kathryn Maitland and her research group at Imperial College London had submitted a manuscript based on the Phase Ib study SEVUSMART to Nature Communications, while the results were made available as a preprint via Research Square pending peer review. The study, conducted at research centers in Kenya and Zambia with funding from Wellcome Trust, established a maximum tolerated dose of 3 mg/kg per infusion and confirmed a favorable safety and tolerability profile in children with severe malaria, meaning a recommended Phase II dose level could be established within a therapeutically relevant range. The deviations in coagulation markers observed at higher doses were clinically silent and normalized within 24 hours. Analyst Group assesses that the study has thereby delivered what it was designed for, and that the dose consistency relative to the level previously identified in adult patients indicates that sevuparin’s safety profile is predictable across different patient populations.
The report further states that the observed 28 day mortality amounted to 4.3% among the 23 children included, compared with an expected mortality risk of at least 10% based on the children’s disease severity and with historical comparison data of 21-34% from the observational study SMAART in the same patient populations. The outcome should nonetheless be interpreted with caution and rather be regarded as an exploratory finding given that the study lacked a comparator arm. That the data is now publicly available is at the same time assessed to broaden the opportunity for both the research community and potential industrial partners to review the results. The next development step, according to the authors, consists of a multicenter randomized Phase II study, whose timing, scope, and funding have not yet been communicated, meaning the timeline within the indication continues to be governed by the priorities of the sponsor and its potential funders rather than by Modus’ own resources. At the same time, this means the Company, together with partners, can conduct the development using non-dilutive funding sources.
Scientific Validation at Leading European Forums
In June, preclinical data for sevuparin in CKD with anemia was presented at the European Hematology Association Congress (EHA2026) in Stockholm and at the European Iron Club Meeting (EIC 2026) in Dublin, in the latter case as an oral presentation. The data covered effects on anemia in an established CKD mouse model, both as monotherapy and in combination with the standard treatment EPO, as well as effects on kidney tissue in the form of reduced fibrosis and improved function. Analyst Group assesses that the presentations constitute an external validation of the scientific basis underlying the ongoing clinical study. That Modus presents data at congresses with a strong presence of clinicians, researchers, and industrial players is further assessed to strengthen the Company’s visibility ahead of partnership discussions, which constitutes a central component of the Company’s strategy for realizing the value in sevuparin through out-licensing.
In summary, Q2-26 was an eventful quarter for Modus, where the Company strengthened both its financial and its scientific foundation ahead of the year’s decisive milestone. The cash position amounts to SEK 12.5m following a strong outcome in TO 2026, the cost level reflects the execution of part 2, and the timeline for the Phase IIa study remains firm with a data readout around the turn of the year 2026/2027. In parallel, the scientific basis has been broadened through the presentations at EHA2026 and EIC 2026 and by the Phase Ib data in severe malaria being made publicly available. Modus further states that structured partner discussions are being held within all three indications, which is in line with the Company’s business model of driving sevuparin through proof-of-concept in order to subsequently realize the value through a licensing agreement or a sale of the Company. Analyst Group views positively that these dialogues are held in parallel with the execution of the study, as this is assessed to be able to shorten the time from data readout to a potential transaction. Overall, Analyst Group assesses that Modus enters H2-26 with maintained momentum and with funding covering the period that is decisive for the Company’s continued development.
We will return with an updated equity research report on Modus.
